I, the parent or legal guardian of the named player above, acknowledge that I am completely aware of the inherent risks associated with this activity, and hereby waive, release and discharge GHP Sport Experiences LLC, West Boca High School, Palm Beach County Board of School and GHP Sport Experiences Corp as well as their officers, directors, coaches, employees and agents (collectively, the “Released Parties”), from any and all liability and responsibility in the event that I or the player become injured in any way while participating in soccer events or activities associated with the Released Parties, including travels to and from training sessions, soccer meetings or other scheduled team activities. I further state that I take full responsibility for any injury that may occur because of my or player’s participation, and that I will not hold the Released Parties responsible for any aggravation of preexisting injuries prior to or during my or player’s participation in any soccer events or activities associated with the Released Parties.

I agree to indemnify and hold harmless the above mentioned organizations and/or individuals, their agents and/or employees, against any and all liability for personal injury, including injuries resulting in death to me, my child(ren) and/or other family members, or damage to my property, the property to my child(ren) and/or other family members, or both, while I or my child(ren) or family members participating in the GHP Sport Experiences LLC, West Boca High School, Palm Beach County Board of School and GHP Sport Experiences Corp activities.

If the participants have a chronic medical condition such as diabetes, seizure disorder, severe allergies or mental health disorder there might be special requirements that are applicable for the athlete to attend to CBF-School Boca programs. Please refrain from registering online and contact us at contact@cbfschoolboca.com to register.

Notice to the minor child´s guardian read this form completely and carefully, you are agreeing to let your minor child in a potentially dangerous activity. You are agreeing that, even if GHP Sport Experiences LLC and or GHP Sport Experiences Corp use reasonable care in providing this activity, there is a chance your child may be seriously injured or killed by participating in this activity because there are certain inherent in the activity which cannot be avoided or eliminated by signing this form, you are giving up your child´s right and your right to recover from GHP SPORT EXPERIENCES LLC, West Boca High School, Palm Beach County Board of School and HP Sport Experiences Corp in a lawsuit for any personal injury, including death, to your child or any property damage that results from the risks that are a natural part of the activity. You have the right to refuse to sign this form and GHP SPORT EXPERIENCES LLC or GHP Sport Experiences Corp has the right to refuse to let your child participate if you do not sign this form.

EMERGENCY AUTHORIZATION:

I, the parent or legal guardian of the named player above, authorize GHP Sport Experiences LLC, West Boca High School, Palm Beach County Board of School and GHP Sport Experiences Corp, their volunteers or representatives to act as my agent(s) to consent to medical, surgical or dental examination and/or treatment at a hospital or other health facility in an emergency where I can not be reached. I agree to be financially responsible for the cost of such assistance and/or treatment. I verify the named player above has no injury, or other conditions which would affect his/her ability to participate, or that I have advised in writing this organization of that condition, including my physician’s name, and confirm the ability of the named player below to participate regardless.

I hereby waive, release and forever discharge GHP Sport Experiences LLC, West Boca High School, Palm Beach County Board of School and GHP Sport Experiences Corp, and associated supervisor, coach or other team administrator from all rights and claims for damages, injury, loss to person or property which may be sustained or occur during participation in our activities, whether or not damages or loss is due to negligence.

IMAGE USE AUTHORIZATION AND WAIVER:

I, the parent or legal guardian of the named player above, hereby grant GHP Sports Experiences LLC, West Boca High School, Palm Beach County Board of School and HP Sport Experiences Corp, its directors, officers, employees, agents, and designees, non-revocable permission to capture the image and likeness of the player named above, in photographs, videotapes, motion pictures, recordings, or any other media (collectively “Images”). I acknowledge that GHP Sports Experiences LLC, West Boca High School, Palm Beach County Board of School and HP Sport Experiences Corp will own such Images and further grant GHP Sports Experiences LLC, West Boca High School, Palm Beach County Board of School and GHP Sport Experiences Corp permission to display, publish, distribute, use, modify, print and reprint such Images in any manner whatsoever related to GHP Sports Experiences LLC, West Boca High School, Palm Beach County Board of School and GHP Sport Experiences Corp business, including without limitation, publications, advertisements, brochures, web site images, or other electronic displays and transmissions thereof. I further waive any right to inspect or approve the use of the Image of the player named above, by the GHP Sports Experiences LLC, West Boca High School, Palm Beach County Board of School and HP Sport Experiences Corp prior to its use. I forever release and hold GHP Sports Experiences LLC, West Boca High School, Palm Beach County Board of School and GHP Sport Experiences Corp harmless from any and all liability arising out of the use of the Images in any manner or media whatsoever and waive any and all claims and causes of action relating to use of the Images, including without limitation, claims for invasion of privacy rights or publicity.

Assumption of the Risk and Waiver of Liability Relating to Coronavirus/COVID-19

The novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization. COVID-19 is extremely contagious and is believed to spread mainly from person-to-person contact. As a result, federal, state, and local governments and federal and state health agencies recommend social distancing and have, in many locations, prohibited the congregation of groups of people.

GHP SPORT EXPERIENCES LLC (CBF SCHOOL BOCA RATON), West Boca High School, Palm Beach County Board of School and GHP Sport Experiences Corp has put in place preventative measures to reduce the spread of COVID-19; however, GHP SPORT EXPERIENCES LLC, West Boca High School, Palm Beach County Board of School and GHP Sport Experiences Corp cannot guarantee that you or your child(ren) will not become infected with COVID-19. Further, attending GHP SPORT EXPERIENCES LLC activities could increase your risk and your child(ren)’s risk of contracting COVID-19.

By signing this agreement, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that my child(ren) and I may be exposed to or infected by COVID-19 by attending GHP Sport Experiences LLC programs and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I understand that the risk of becoming exposed to or infected by COVID-19 at GHP Sport Experiences LLC may result from the actions, omissions, or negligence of myself and others, including, but not limited to, GHP Sport Experiences LLC employees, volunteers, program participants and their families and West Boca High School, Palm Beach County Board of School and GHP Sport Experiences Corp.

I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to my child(ren) or myself (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that I or my child(ren) may experience or incur in connection with my child(ren)’s attendance at GHP Sport Experiences LLC or participation in GHP Sport Experiences LLC activities (“Claims”). On my behalf, and on behalf of my children, I hereby release, covenant not to sue, discharge, and hold harmless GHP Sport Experiences LLC, West Boca High School, Palm Beach County Board of School and GHP Sport Experiences Corp, its employees, agents, and representatives, of and from the Claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto. I understand and agree that this release includes any Claims based on the actions, omissions, or negligence of GHP Sport Experiences LLC, West Boca High School, Palm Beach County Board of School and GHP Sport Experiences Corp, its employees, agents, and representatives, whether a COVID-19 infection occurs before, during, or after participation in any GHP Sport Experiences LLC activities.
